Location: North Kansas City, Missouri
Functions:
· Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to the patient/family/caregiver.
· Is responsible for the implementation and continued development of the patient plan of care (PPOC) and physician's plan of treatment
· Provides ongoing patient and family teaching. Assesses continuing teaching needs of patient/family/caregiver.
· Is responsible for the Admission, case management and discharge of the patient, including symptom management in collaboration with the physician and other team members.
· Check temperature of medication refrigerator and logs results daily.
· Documents care accurately in the Medical Record. Communicates with physicians and others as needed to coordinate and. ensure high quality care.
· Communicates on a regular basis with all staff and volunteers regarding patient status.
· Attends and participates in daily report, team meetings, briefings, in-services, department and staff meetings and other related activities, as appropriate to the work schedule
· Understands and adheres to the agency's mission, policies and procedures.
· Works effectively in-an interdisciplinary team environment and assists throughout the facility.
· Follows the guidelines for universal-precautions when providing patient care.
· Supervises and collaborates with LPN's and PCP's. Serves as lead nurse when assigned, coordinating and. resolving issues.
· Works effectively with homecare teams in admissions and discharges
· Demonstrates a positive attitude and working relationship with staff-members and others.
· Carriers out duties and responsibilities of position in a manner that exemplifies excellent customer service.
· Consistently represents the Facility to all internal and external customers with integrity and. Professionalism
· Maintains strict confidentiality of patient/family/caregiver information.
· Provides for a safe environment for patients, guests, visitors and employees.
· Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security and risk management
· Maintains positive work relationships and contributes to at team. environment by:
Demonstrating respectful and appropriate language and nonverbal behavior,
Actively seeking to resolve customer and co-worker conflict issues, problems and concerns in a timely manner; and
Demonstrates respect, courtesy and empathy to all individuals; patients; families, co-workers, physicians and volunteers.
