Heritage OnCare Home Health is currently seeking an experienced Home Health Assistant- CNA to join our rapidly growing team.
The Home Health Assistant (HHA) provides direct client care services primarily delivered on a unit basis in the client's home and or place of residence. The HHA will promote the growth of Heritage OnCare Home Health by representing the organization, its mission, and its role in the community in a professional manner.

Responsibilities may include:


  • Knowledgeable of patient/client legal rights, respects patient/client rights always, and reports the failure of any other person to comply with patient rights including abuse, neglect, or disrespect.

  • Maintains confidentiality about all patient's personal, financial, and health information including following HIPAA guidelines. Does not share agency issues, concerns, or problems with patients/clients/customers or other entities.

  • Deals tactfully with team members, patients, families, visitors and government agencies/personnel.

  • Must possess the ability to make independent safety decisions when circumstances warrant.

  • Understands and utilizes universal precautions, utilizes and maintains equipment and requests supplies as needed.

  • Performs timely documentation per agency policy, for each client visit.

  • Home Health Assistant will comply with state regulations regarding supervisory visits per regulation for HHA scope.

  • Reviews the plan of care at every visit and will continuously be reporting the client's status and reporting changes, communicating with the client/caregiver/facility team members, case manager and working with all involved to ensure all needs are met.

  • Will provide care that is only in their scope of service which includes: measuring blood pressure, temperature, pulse, respiration, weight. Transferring clients using wheel chairs, walker, cane, or assisting them to ambulate per plan of care.

  • Assisting of client per agency policy, plan of care or as directed by the case manager may include but not limited to: Bathing, toileting, hair care, applying lotion, nail and skin care, shaving, dressing, oral cares, safe transfers and assisting with ambulation, normal range of motion and positioning. Cleaning of bathing area and properly storing equipment and supplies once all tasks have been completed if noted on plan of care.


Qualifications:

  • Must have completed the required state law certification of 75 hours for CNA in the State of Nebraska.

  • Must possess a current CPR certification

  • Three years' experience as a HHA preferred

  • Prior Home care experience preferred

  • Ability to provide care in a variety of settings

  • Three years' experience as a HHA preferred

  • Must be at least 18 years of age
