Community Health Navigator - Temporary at Catholic Charities

The Community Health Navigator works in collaboration with the COVID-19 Response Initiative community partners (Esperanza Center Health Services, Johns Hopkins Centro SOL, BUILD Coalition, Mayor's Office of Immigrant Affairs). The project will run from November, 2020 to March 31, 2021. 

The Community Health Navigator is responsible for:


  • In coordination with Johns Hopkins Hospital, delivers COVID-19 test results to patients via phone and letters.

  • Provides guidance regarding isolation for patients diagnosed with COVID-19.

  • Identifies and develops relationships with social service agencies for Latino immigrants in Baltimore City, Baltimore County, and Anne Arundel County

  • Prepares and delivers home safety kits to impacted households to include face masks, hand sanitizer, and other items deemed necessary.

  • Documents all patient encounters in EPIC, a healthcare tracking system, and other HIPAA-secured platforms as required

  • Staff the Esperanza Center COVID-19 Hotline to ensure community access to clinical guidance, COVID-19 follow-up care, and access to testing.

  • Provide COVID-19 test results to patients in a timely manner, assessing social needs and engaging patients to services such as food delivery, cash assistance and referral to isolation locations where needed.

  • Provide education on COVID-19, isolation protocols, contact tracing, and refer contacts to testing as needed.    

  • Maintains professional/personal boundaries with patients, keeping the Program Coordinator apprised of all situations of possible conflict, and maintain strict confidentiality of all patient information.

  • As needed, assist with COVID-19 community testing events in person to ensure appropriate language and cultural capacity is present.  

  • As needed, assist with Diabetes-related education and awareness in the Latino community.

  • Prepares and processes invoices, check requests, field expense vouchers and other documents related to program expenses (travel, food, material purchases, etc. ) as appropriate.


This position requires:


  • High School Diploma.


  • Bilingual in English and Spanish required.

  • Life/community/work experience or knowledge regarding the Latino Community and infectious diseases (COVID-19 and Diabetes) preferred.

  • One Year experience working with a culturally diverse population required.

  • Working knowledge of community resources and ability to connect families to community services.

  • Ability to learn and master database software. 

  • Requires the ability to utilize computer systems and software necessary to perform position functions. Basic Windows PC, web browsing (i.e. Chrome, Internet Explorer, etc.), and Microsoft Outlook skills required. Other Microsoft Office application knowledge, such as Word, Excel PowerPoint, Skype, and OneDrive desired.

  • Must have reliable transportation to visit multiple sites. If driving on Agency business, must be at least 21 years of age and have valid driver’s license issued by the state of residence, with no more than 3 points.
