Job Description:

The Senior Manager, Health Homes Program provides leadership, oversight, and direction to the Health Homes Program, which engages and supports members dealing with a confluence health conditions, behavioral health challenges, and social needs. With direction from leadership, they will facilitate the implementation and operation of the Health Homes Program, ensuring all elements are in compliance with health plan and regulatory requirements as well as improving towards optimal performance. The incumbent will also represent the Plan externally along with leadership regarding Health Homes at local and state forums. The incumbent will serve as a resource and educator for the Health Homes Program

Essential Functions and Key Responsibilities:
Leadership

Support, communicate, and reinforce the mission, vision, values, philosophy and culture of the organization

Ensure best utilization of resources in obtaining organization goals, regulatory compliance, with corporate policies and to ensure the needs of the individuals served are met.

Establishes and maintains positive relationships with local and state government, community leaders, pastors, congregations, county social service personnel, professional associations and other service providers as necessary in the overall management of the program.
-
Engages in other outside activities consistent with the organization's responsibilities in the community and the industry.
-
Anticipate and effectively deal with issues that could enhance the region's opportunities or adversely impact business results.
-
Demonstrate a passion for leading positive change, comfortable creating new processes or building on best practices.

Works with Healthcare Informatics and Quality to explore new technologies to deliver innovative, user friendly, integrative interface design solutions across multiple platforms to ensure that interventions and approaches are holistic and member-centered.

Program Development and Implementation

-
Oversee the implementation of the Health Homes Program.
-
Provide operational and performance oversight to Health Homes Program operations both internally and with contracted community partners
-
Develop internal expertise and technical assistance to internal and external stakeholders - Ensure optimal operations and continuous improvement of the Health Homes Program.

Monitor progress and performance of the program
Department Management

-
Serve as resource for Health Home Program.
-
Mentor, guide and coach direct reports in order to expand their capabilities and build management depth in the organization.
-
Directly responsible for management level professionals with ultimate accountability of clerical and other non-management professional reports.

Responsible for staffing, monitoring productivity, and performance of - staff.
-
Ensures new employees are oriented to their department as well as trained for their jobs. - Organizes the department workload and establishes work standards to promote efficiency and productivity.

Oversees prepares and conducts fair and objective performance appraisal and goal setting for staff in department.
Regulatory Compliance

-
Attend all mandatory management education and training courses.
-
Serve as single point of contact for Health Homes Program reporting
-
Understands and abides by all departmental policies and procedures as well as the organization's Standards of Conduct and Corporate Compliance Program.
Experience:

-
Experience working with Medi-Cal populations.
-
Thorough knowledge of case management and social work principles, techniques and practices and their application to complex casework and community problems
-
Skill in implementing operational aspects of programs and policies with ongoing process improvements
-
Leadership skills that includes team building and conflict resolution
-
Experience working with people who have complex health and social issues.
-
Experience partnering with provider groups and community resources and agencies.
-
Experience working with state and local government stakeholders.

Education:

-
Bachelor's Degree in applicable fields
-
Master's Degree preferred - Applicable clinical licensure in social work preferred License/Certification/Special Training:

Valid CA Driver's License and proof of auto insurance coverage required.

Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork - Activity level: Sedentary, frequency most of work day.
