The Health Care Quality and Affordability team is made up of about 1,000 doctors, nurses, pharmacists, strategists, industry experts and innovators with a single vision - to transform the delivery of health care. We continue to expand our network of providers to ensure state-wide access to care. We evaluate analytic and digital health tools that simplify care delivery while improving member health status, health care quality and affordability as well as member and provider experience. We provide high-touch services to members from experienced clinical teams. You'll also find us collaborating with our federal and state legislative officials to help shape policy to advance our Blue Shield mission.

Position Summary:

Reporting to the Social Services Manager, the role of the Community Health Advocate is critical to the success of Blue Shield of California and Promise Health Plan in realizing its goals and objectives related to improving access to care. The Community Health Advocate addresses social determinates of health by providing care coordination for members with complex health conditions, behavioral health challenges, and social needs. With direction from leadership, and connection to the social service and larger internal teams, the Community Health Advocate assists members with linkage to appropriate medical, social and community resources. The incumbent also supports individual and community engagement by increasing community resource knowledge through a range of activities such as outreach, community resource linkage, informal counseling, social support and health advocacy.

Specific Responsibilities Include:

- Responsible for establishing trusting relationships with members while providing general support and encouragement.

- Conduct social determinants of health needs assessment and/or augmented information obtained from the social determinants screening tool.

- Provide referrals and linkage to community agencies.

- Provide field-based and telephonic on-going follow up and support to members accessing community agencies and services.

- Establish strong relationships with community partners.

- Participate in local community engagement activities with local agencies where members present such as faith-based organizations, community centers, government agencies, parks and recreation centers, and schools.

- Actively participates with the interdisciplinary team and provides updates on members' ongoing needs and progress with goals.

- As a best practice, actively participates in weekly or bi-weekly case consultation with a Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW).

- Prepares and updates productivity reports, as needed.

- Documents case notes in the clinical case management documentation system within the established guidelines.

Minimum Education & Experience Requirements:

-

High School diploma or equivalent.

-

A minimum of 1 year experience working with populations impacted by health and social issues.

-

A minimum of 1 year experience navigating, linkage to, and interacting with community-based agencies and governmental systems.

-

Valid CA Driver's License and proof of auto insurance coverage required.

Preferences:

-

Bachelors Degree in Social Work or equivalent preferred.

-

Experience and or training in motivational interviewing.

-

Experience working with Medi-Cal populations.

-

Bilingual/Spanish speaking preferred.

Knowledge and Skills:

-

Considerable knowledge of the wide range of psychosocial challenges.

-

Knowledge of family and group dynamics along with relevant intervention techniques.

-

Knowledge of health disparities in diverse communities.

-

Excellent communication skills both orally and in writing with all levels of BSC Promise staff, members, contracted physicians, and participating provider groups.

-

Excellent rapport and trusting building skills with a disenfranchised population.

-

Effective time management skills and ability to define and act on priorities efficiently.

-

Excellent analytical skills.

-

Must have proficient skills with computer programs, including Microsoft Excel, Outlook, Word, and PowerPoint.
