The Health Care Quality
and Affordability team is made up of about 1,000 doctors, nurses, pharmacists,
strategists, industry experts and innovators with a single vision - to

network of providers to ensure state-wide access to care. We evaluate analytic
and digital health tools that simplify care delivery while improving member
health status, health care quality and affordability as well as member and
provider experience. We provide high-touch services to members from experienced
clinical teams. You'll also find us collaborating with our federal and state
legislative officials to help shape policy to advance our Blue Shield mission.

Position
Summary:

Reporting to the Manager of Social
Services, the role of the Health Advocate, health homes is critical to the
success of Blue Shield of California and the Promise Health, Medi-Cal division
in realizing its goals and objectives related to the Health Homes Program. The Health Advocate, Health Homes Program provides
field-based outreach, engagement and supports members dealing with complex health
conditions, behavioral health challenges, and social needs. With direction from leadership, and
connection to the social service team and larger internal teams, the health
advocate will assist members in connection to appropriate medical, social and
community resources.

Specific
Responsibilities Include:

- Provides
in person support to members and/or family members who meet the criteria for
engagement into the health.
- Responsible
for establishing trusting relationships with members while providing general
support and encouragement.
- Conducts
social determinants of health needs assessment and/or augmented information
obtained from social determinates screening tool.
- Provide
referrals and linkage to community-based services.
- Provide
on-going follow up and support to members accessing community agencies and
services.
- Establish
strong relationships with community partners.
- Participate
in local community engagement activities with local agencies where members
present such as faith-based organizations, community centers, government
agencies, parks and recreation centers and schools.
- Actively
participates with the interdisciplinary team and provides updates on member's
ongoing needs as well as their progress.
- As
a best practice, participates in weekly or bi-weekly case consultation with the
Licensed Clinician.
- Prepares
and updates productivity reports as needed.
- Documents
case notes in the clinical case management documentation system

Minimum
Education & Experience Requirements:

- A minimum
of an associate degree in behavioral health, social work or related
degree.
- A minimum
of 1 years' experience community service or other related

Preferences:

- Bachelor's
degree in behavioral health, social work or other related degree
- Experience
and or training in motivational interviewing
- Experience
with Medi-Cal population

Knowledge and Skills:

- Skill
in establishing rapport with members Ability to plan and execute work effectively
Ability to express ideas clearly and concisely
- Effective
time management skills and ability to define and act on priorities efficiently
Excellent analytical skills
- Must
have proficient skills with the company computer systems, including Excel, Word,
and Power Point
- Excellent
communication skills both orally and in writing with all levels of BSC Promise
staff, members, contracted physicians, and participating provider groups

- Physical Requirements:
Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork - Activity level: Sedentary, frequency most of work day.
