Scribe Trainer - Remote CA at Vituity

Job Description:

Vituity, among the largest truly democratic physician partnerships in the nation, has a paid part-time position as a Medical TeleScribe Trainer for students interested in a career in healthcare, specifically those with intention of attending medical, physician assistant, or nursing school.

A TeleScribe Trainer helps providers expedite patient throughput times as well as improve clinical documentation. A TeleScribe Trainer is an unlicensed person who enters information into the electronic medical record (EMR) or chart at the direction of a physician or practitioner. A TeleScribe Trainer functions under the direct, remote supervision of the provider. TeleScribes also assist the provider in navigating the EMR and in locating information such as test results and lab results. The TeleScribe will also facilitate preparation of patient appointments through direct communication with the site. The TeleScribe will prepare charts prior to the provider's patient encounters, serving to support the work flow and documentation for medical record coding. TeleScribes can also support the physician or practitioner by reminding them of quality measures that need to be documented and ensuring that diagnosis documentation is accurate to ensure proper reimbursement.

Workflow and Documentation


  • Contact site to confirm that remote TeleHealth computer is in the correct location with the correct patient, so the Provider has it technology readily available.
  • TeleScribe Trainer will ensure that audio/video capabilities of the TeleHealth Workstation are functioning effectively
  • Prepare patient charts with necessary documentation and history
  • Transcribe the history, physical exam, laboratory, and radiological results as dictated by the provider. This may include selecting the appropriate template from the computer system. In certain departments they can assist with documenting H&Ps, Progress Notes and Discharge Summaries.
  • Facilitate the acquisition of diagnostic test results from ancillary departments. Compile results for the provider's review and action. Record test results on the medical record as directed by the provider.
  • Record, in real time, the actions (physical exam and procedures) and words of the provider. Obtain the provider's signature on every chart. Record the discussions that occur between the provider and medical consultants or patients' family members.
  • Ensure complete and legible documentation of the patient encounter and notify provider if more information is needed for documentation. This may include reviewing triage and nursing documentation for completeness.
  • Review the primary diagnoses and work with the provider to ensure that all documentation is accurate and is clinically supported in the charting. Work with the provider to ensure that all quality measures are completed.
  • May pend orders under the discretion of the provider if the hospital/site permits this practice

Education/Work Experience Requirements

Minimum Required Qualifications:



    • High school diploma or GED equivalent required
    • Strong writing skills; legibility and spelling proficiency required.
    • Professional demeanor with strong interpersonal and communication skills; must be a team player.
    • Excellent organizational skills.
    • 1+ year commitment
    • Self-motivated
    • Excellent follow through

Preferred Qualifications:



    • Pre-med or nursing related courses preferred (upper division Anatomy or Physiology is a plus).
    • Knowledge of medical terminology a plus.
    • Solid computer skills; experience with EMR is a plus.

Competencies (Knowledge/Skills/Abilities):



    • Quick and accurate typing speed (minimum 45wpm)
    • Intermediate proficiency in a Windows PC environment (Excel, Word, OneNote, Outlook, et cetera)
