Position description

Under the supervision of a social worker who meets state required standards, provides counseling, and other services in collaboration with multidisciplinary teams. The social worker may also coordinate patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff within the section of Social Work.

Collaborate with primary care extended care team to address needs of patients.
Address psychosocial issues that may be contributing to increased emergency room visits and/or hospitalization
Identify patients who do not have a completed Power of Attorney for Healthcare and work with identified patient to complete documents.

Will provide some support to clinic in Glenwood City and Elmwood.

Qualifications

An MSW degree is required from an accredited Masters of Social Work Program.

Additional qualifications

Must have an advanced knowledge of categories in mental illness, personality development, knowledge of the types of psychiatric treatment, and skills in conducting individual, group, and family therapy. Must have knowledge in adjustment to medical diagnosis and chronic disease. Strong human relation skills are needed to interact with patients, patient's family members, consultants, residents, hospital personnel, community agencies, and other resources. Must also have psychosocial diagnostic skills and the ability to communicate effectively in both verbal and written form.

License or certification

Wisconsin: Certification at the Certified Advanced Practice (CAPSW) or Certified Independent Social Worker (CISW) level by the Wisconsin Department of Safety and Professional Services is required. CAPSW personnel who have achieved two years of supervised social work practice, by a supervisor who has met board requirements, may apply for certification as a CISW. CAPSW or CISW personnel, following 3,000 hours of clinical social work practice including at least 1,000 hours of face to face contact supervised by a person meeting board requirements, may apply for licensure at the LCSW level. Valid driver's license required for specific areas.
