KMHS is Kitsap’s nonprofit, designated community mental health provider for adults with acute and/or chronic serious mental illnesses, and children with serious emotional disturbances and their families. During 2016, KMHS provided services to more than 6,800 children and adults. PURPOSE:

To provide support and assistance to KMHS Care teams, clients and their families so as to ensure the effective and efficient coordination of services with primary, specialty, and other health care, strengthen targeted chronic disease prevention and management activities, and track client health data in the client health record.

To improve the operations of KMHS in support of its stated mission. To provide the highest level of customer service to internal and external customers.

RESPONSIBILITIES AND DUTIES:


  • Work with Child & Family (C&F) teams, community health providers and hospitals to establish systematic linkages and communications so as to ensure the timely exchange of relevant health data and facilitate the coordination of behavioral and physical health care.

  • Provide health promotion services and activities to KMHS clients and families including conducting educational groups to improve their understanding of health and self-management capabilities.

  • Provide consultations to C&F team staff regarding identified youth and families who would benefit from information and coaching regarding healthy behaviors, wellness, nutrition, and other health factors specific to overall family health and management medical conditions.

  • Assist the C&F teams with timely communications with primary and specialty medical care providers, acute care hospitals, and emergency departments regarding identified clients.

  • Communicate key client health information and needs to C&F teams for comprehensive care coordination and treatment planning.

  • Assist with gathering and tracking health data including accurate and timely entering of data into the electronic health record and other relevant health data systems.

  • In conjunction with the Healthy Living Program Developer (HLPD), develop assessment, coaching and educational tools and resources targeted to identify chronic disease prevention and management needs of C&F clients and their families.

  • In conjunction with the HLPD, provide support to C&F teams to interpret, prioritize and present client health data to the teams, with assistance from C&F nursing staff and Psychiatric Medical Providers.

  • Work closely with HLPD, nursing and psychiatric providers, and PCPs to stay abreast of client health needs, developing evidence based interventions that contribute to positive health outcomes, and guide the development of KMHS health related services.

  • Other duties as assigned.

RESPONSIBLE TO: Outpatient Team Supervisor

MINIMUM QUALIFICATIONS:

BA in Health or Behavioral Health related field with child and adolescent medical/wellness experience. Knowledge of chronic health conditions affecting children and impact of family health habits. Experience working in a health care setting providing health education to youth and families preferred. Ability to work within interdisciplinary team setting on behalf of complex clients with extensive behavioral care needs. Experience with EMR and data tracking and ability to read data reports. Familiarity with outpatient medical clinic operations so as to facilitate the establishment of positive and productive relationships with primary care staff throughout the community. Effective collaboration and communication skills. Requires valid WA State Driver’s License and reliable, insured transportation. Possess and maintain valid driver’s license with an acceptable motor vehicle report.

PHYSICAL CONSIDERATIONS: While performing the duties of the job, the employee is required to walk, sit, use hands and fingers, reach with arms, talk or listen. Peripheral vision is also required for this position.
