Job Description:

SUMMARY

A person in this position will serve individuals diagnosed with severe emotional disturbances and/or developmental disabilities. Provides skill building with children and their families, individually or within a group setting. Provides services within the home, school and/or other community settings. Provides skill building in a variety of areas which may include: social skills, skills of daily living, and emotion regulation. Persons with Lived Experience with mental illness, intellectual/developmental disabilities or substance abuse disorders are encouraged to apply.



JOB DUTIES AND RESPONSIBILITIES

Complete direct contact with the consumer as specified in the Plan of Service.
Document contact/services, complete billing procedures per policy/procedures.
Work collaboratively with a multi-disciplinary team.
Work in collaboration with other agency programs, community agencies to serve the needs of the consumer as identified in the Plan of Service.
Knowledge of Medicaid Guidelines and Level of Care Guidelines.
Respond to Emergency needs of consumers during crisis situations.
Work flexible schedule i.e. weekends, evenings, and holidays if deemed necessary.
Travel as needed to meet consumers and administrative needs.
Complete direct contact with consumers in diverse community settings.
Ability to work with fellow staff, clients and other agency professional on an on-going basis.
Demonstrates the ability to present one's self, the agency and the individuals we serve in a positive manner at all times.
Assure the maintenance of departmental case records in compliance with DCH, CMH, Medicaid and other applicable standards.
Participates in group facilitation that reflect needs identified in the Plan of Service.
Acting as an advocate for the families.
Demonstrate knowledge of, follow and utilize agency policies, procedures and standards in the provision of services to consumers.
Be sufficiently aware of Community Mental Health programs to assist in orientation and training of new staff and involvement in community education as needed.
Other duties as assigned.



COMPETENCIES

Possess basic computer and organizational skills.
Accurately enter daily documentation into the electronic medical record.
Be able to read, write and formulate narrative documents.
Holds knowledge of and can effectively operate AVATAR electronic programs, and employee email within the agency email system.
Must possess good verbal and social skills with the ability to engage consumers and their families in treatment.
Be able to drive to and from various work locations.
