Leads the transformation to the value based payment model for Community Care Management Partners. Oversees the quality of services provided and ensures the program is in compliance with applicable governmental, regulatory and contractual requirements. Supports the Director in overseeing appropriate utilization and management of staff to promote quality care consistent with the goals and mission of the program. Serves as primary liaison to IT and billing vendors. Designs and implements policies in order to meet quality and regulatory needs. Serves as Health Home subject matter expert for the CCMP network. Works under general direction.

Education: Bachelor's Degree required. Master's degree in Health Administration, Public Health, Social Work, or other related field preferred.

Experience: Minimum of five years of relevant experience of progressively increasing responsibility required. Relevant experience may include: Case Management, Care Coordination or other direct service activities, the administration and management thereof; Quality Management, Quality Assurance, Compliance and/or Program Operations activities; and/or data driven management in the healthcare, mental health, or social services sectors. Strong interpersonal and leadership skills required. Excellent oral, written and interpersonal communication skills required. Knowledge of Microsoft applications required.
