Job Description:

The mission of the Red Sox Foundation and Massachusetts General Hospital Home Base Program is to heal the invisible wounds of war, including post traumatic stress disorder (PTSD) and traumatic brain injury (TBI), by connecting Veteran, Service Members, and their families to world class care. Home Base provides clinical care and support services, as well as community education. Home Base operates an Outpatient Clinic, a two-week Intensive Clinical Program supporting veterans on a national scale, as well as other wellness and resilience programs. Several pilot clinical programs are also being launched in the coming year.

Home Base is currently searching for a half- to fulltime Psychiatric Nurse Practitioner to provide care to individuals suffering from the invisible wounds. Notably, a significant portion of the population we serve are treated for co-morbid conditions including Traumatic Brain Injury and Substance Use. This role will work with the clinics inter-disciplinary team to provide compassionate care to patients treated in all clinical services at Home Base including: Intensive Clinical Program, Outpatient Clinic and Traumatic Brain Injury Service. Responsibilities will include evaluation, diagnosis, treatment, education, and management of the substance use disorder and co-occurring medical and psychiatric illnesses.

PRINCIPAL DUTIES AND RESPONSIBILITIES: Indicate key areas of responsibility, major job duties, special projects and key objectives for this position. These items should be evaluated throughout the year and included in the written annual evaluation.

Patient Admission Process:

* Will work in collaboration with the Medical and Clinical Director as part of the intake process for all clinical Home Base programs.

* Will be responsible for gathering and reviewing medical data, communicating with primary care and other health care providers about potential patients.

* Will assess patients presenting under the influence at Home Base, and communicate the assessment result with physician and/or other clinical team members.

* Will facilitate (when necessary) the transfer of the patient to the Emergency Department, Acute Psychiatry Service or to a primary care practice.



Clinical Care:

* Provides clinical visits.

* Develops and implements disease-specific patient management plans, records progress notes, and assists in provision of continuity of care.

* Assesses, instructs and counsels patients regarding compliance with treatments.

* Orders, interprets, and evaluates diagnostic tests to identify and assess patients clinical problems and health care needs.

* Records physical findings, and formulates plan and prognosis, based on patients condition.

* Prescribes or recommends other forms of treatment.

* Maintains accurate and timely notes in the electronic medical record.

* Provide comprehensive care in accordance with a collaboration agreement with a supervising psychiatrist.

* Self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency within specialty of Psychiatry.

* Performs other duties as assigned.

* Ability to prescribe suboxone preferred


Facilitating Continuity of Care: Oversees and collaborates as necessary in the care of clinic patients.

* Participates in and facilitates inpatient care for Home Base patients admitted to hospital.

* Explore innovative care models.

* Collaborates with outside facilities and clinical staff for certain patients, and also to provide general information about caring patients to other caregivers including outside hospitals, PCPs, VA and private help.



Other:

* Provide support and assistance with challenging patient/physician situations.

* Actively participate in network development initiatives and communicate initiatives back to access team for relationship building within MGH affiliate networks.

* Participate as requested in MGH and Partners quality initiatives.


SKILLS/ABILITIES/COMPETENCIES REQUIRED: Must be realistic, objective, measurable and related to essential functions of this job.

* Completes biopsychosocial and psychopharmacological evaluations.

* Develops treatment plans based on assessment and provides appropriate treatment.

* Maintains working relationship with patient and/or family while maintaining appropriate professional boundaries.

* Maintains effective working relationships with collaborative leadership teams, interdisciplinary health care teams, and appropriate community providers.

* Documents in the medical record according to departmental policies and procedures

* Able to work flexibly and at the pace required in an Intensive Outpatient Program

LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable): Specify minimum credentials and clearly indicate if preferred or required
