The Director of Clinical Services is responsible for ensuring that patient care is coordinated and managed appropriately. Oversee all Clinical Staff of Home Health agency, including, but, not limited to: RN Case Managers, LPN s, Home Health Aides, and Therapists.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Receives case referrals; reviews available patient information related to the case, including home visits, to determine home care needs; assigns appropriate home health personnel to case as needed.
Reviews and evaluates each case through a variety of means such as home visits, case conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.
Reviews patients medical diagnosis, prognosis, medications, procedures and clinical course.
Assists clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plan of care.
Attend and/or lead team meetings and case conference meetings with home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals; assists in the orientation of new employees.
Collaborates with other supervisory personnel in the planning, implementation and evaluation of in-service and continuing education programs. Participates in the formulating, revising, implementing and evaluating organization policies, procedures, goals, and objectives.
Assesses health needs of the community and assists in the development of plan(s) and/or program(s) to meet needs; participates in public relation and community activities that promote the organization s role as an effective member of the home health care delivery system.
Participates actively in quality assessment performance improvement teams and activities in accordance with the Quality Management Program.
COMPANY WIDE RESPONSIBILITIES
Supports a dignified and caring atmosphere with Home Health staff and field staff. All information obtained in the field, or at Home Health Agency, is kept strictly confidential.
Current registered nurse (RN) license in the state of Washington or Physical Therapist (PT) in the state of Washington
Must have a proven ability to work within an interdisciplinary setting
Must have a current driver s license
Must have excellent written and communication skills
EDUCATION and/or EXPERIENCE
Graduate of an NLN accredited school of nursing with a BSN degree preferred
Graduate of an accredited school of Physical Therapy preferred
3 to5 years of managerial experience preferred
3 to5 years experience in home care setting preferred
