Primary Purpose: Ensure medical information is available and accessible through accurate and timely medical record processing and retrieval functions. Prepare and scan medical record documents into the electronic document management system, in accordance with department and hospital policy and procedures.
Prepare and scan medical record documents into the electronic document management system, in accordance with department and hospital policy and procedures.
Retrieve and reconcile every Inpatient, ED, Observation and Outpatient record
Sort loose medical record documents into appropriate record
Follow up on missing records to ensure timely processing
Prepare records and batches for scanning according to procedure
Scan all documents into the Streamline and Epic systems according to procedure
Assess the quality of all scanned documents while scanning into AccessANYware and facilitate re-scanning if appropriate according to procedure
Store boxes of scanned documents until the QA procedure is complete
Maintain record storage area so the records are neat, orderly and accessible
Communicate system and/or user concerns to manager
j. Process clinic records and perform indexing of documents.
Knowledge and Skills
Education: High school diploma or GED.
Years of Experience: Excellent organizational and communication skills. Computer experience, medical terminology preferred.
