The following statements are intended to describe the general nature and level of work being performed and are not intended to be a complete list of all responsibilities, duties and skills required.
Provides services and care for residents and patients. Oversees new resident arrivals, orients residents and family to facility accommodations and helps with the discharge process. Personally tracks each new resident daily, to ensure their care and comfort needs are met. Additionally, provides support to Nursing Home Administrator and Director of Nursing relating to customer service initiatives and helps provide resolution with the resident/family grievance process.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Coordinates with the necessary departments to ensure room is ready for new admission: bed is made, soap, washcloth and towels are in the room, and hangers in closet, water picture and cup are accessible.
Greets new resident upon arrival and provides all necessary introductions.
Accompany resident to room and ensures resident is comfortable prior to exiting.
Verifies diet order with Licensed Nurse and offers bedside fluids as well as meal tray if admission is at meal time
Orients resident and family to their new environment by providing a tour of the facility.
Provides immediate personal care and services upon arrival such as: a) Assisting with activities of daily living (turning and positioning, toileting, bathing and grooming, feeding, and mobility)
b) Obtains and records vital signs and weight as assigned.
c) Accurately documents all care as required by company policy and in compliance with state and federal regulations.
Marks resident’s dentures, glasses, hearing aid, artificial limbs, clothing and other personal belongings.
Completes Inventory of Personal Effects form and places in medical record under Legal / Risk tab
Initiates the C.N.A. ADL flow record
List all known allergies.
Reports any skin condition to nurse.
Takes a picture of the new resident.
Provides the new resident with the welcome basket
Coordinates with Nursing and Admissions Department that the resident and/or responsible party signs all admission forms on day of arrival.
Contributes to the resident assessment and care plan.
Promptly communicates any significant changes in behavior or condition to supervisor.
Completes required forms and documents in accordance with company policy and state and/or federal regulations.
Upon discharge, pack all belongings neatly and escorts resident and family to their vehicle, if applicable, otherwise coordinates for proper storage of resident’s belongings.
Follow up with Social Services regarding discharge plan and coordinates required home medical needs.
Informs the Nursing and Housekeeping Department of the discharge.
Successful completion of State-approved Nursing Assistant Certification course.
Must have 3+ years healthcare experience, preferably in the long-term care environment.
Must have excellent verbal and written communication skills.
Must demonstrate an outgoing, energetic personality.
Must be willing to take floor C.N.A. assignments as needed.
PHYSICAL DEMANDS AND ENVIRONMENT
Shift work may be required. Employee is frequently required to stand, walk, lift and or transfer residents weighing between 100 and 250 pounds, see, use hands or fingers, reach with hands and arms, feel, talk and hear. Employee is occasionally exposed to blood or other body fluids, fumes or airborne particles and toxic or caustic chemicals. May be required to work beyond their regular scheduled shift. Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident’s protected health information. Monitors and ensures compliance with company policies, procedures and state and federal law.
