Job Description:
With minimal supervision, provide technical support to SNF Program clinician staff, SNF coordinators and SNF placement coordinator. Coordinate all patient stay data for Kaiser Permanente members residing in skilled nursing facilities. Provide administrative support for SNF Program.

Essential Responsibilities:
  • Manage, evaluate and verify accuracy of information in multiple software applications for all Kaiser Permanente members residing in licensed Skilled nursing facilities. Process all incoming referrals for skilled nursing, verifying benefits and eligibility. Effectively communicate to team members, the member's level of care and benefit status. Update and research questions/queries made by department members (physicians, nurse practitioners, physician assistant, care coordinators, utilization managers) processing admits/discharges in the access database and Epic Healthconnect. Concurrently research accuracy of member data to ensure reliability, validity and integrity of information received from various facilities and other Kaiser Permanente departments. Manages and verifies accuracy of out-of-area, home SNF benefit and out-of-contract facility agreements and authorize referrals in TAPESTRY for all SNF admits.
  • Design, maintain, and continuously improve reporting systems for SNF program including, but not limited to, Navi Health, QM satisfaction surveys and SNF census.
  • Assure timely communication of information to the SNF Program supervisor and clinical operations manager for financial and utilization analysis. Maintains filing system for SNF program. Data entry of NaviHealth statistical information. Run reports as needed. Answer multi line phone calls and triage to appropriate staff or department.
  • Acts as a resource to supervisors, clinicians, and support staff to meet information needs. Determine patient current benefit coverage using CMS and report finding to Supervisor and SNF care coordinators. Provide administrative support to all members of the SNF Program. Maintain Access database for Web site directory for SNF. Attend departmental meetings. Provide administrative support to the, SNF Clinical Manager and SNF Supervisor.

    Basic Qualifications:
    Experience
  • Minimum two (2) years of experience in data entry or clerical work.
    Education
  • High School Diploma or General Education Development (GED) required.
    License, Certification, Registration

    Additional Requirements:
  • Working knowledge of PC-based computer systems; (Microsoft) word processing, spreadsheet application and report design in Excel.
  • Demonstrates customer-focused service skills.
  • Ability to focus attention and complete critical tasks despite frequent interruptions over entire workday.
  • Excellent organizational skills.
  • Ability to transfer large volumes of information with a high degree of accuracy.
  • Ability to analyze accurately, problem-solve, and make appropriate decisions.
  • Typing 30 WPM.


    Preferred Qualifications:
  • Minimum four (4) years in health care environment.
  • Thorough knowledge of Kaiser Permanente organization and structure.
  • Working knowledge of Kaiser Permanente systems.
  • Medical terminology course.
  • Working knowledge of Access.
  • Demonstrated ability in 10 key.

  • Associate's degree in Business Administration.
